Pneumococcal disease is a leading cause of serious illness in children and adults throughout the world. The disease is caused by a common bacterium, the pneumococcus, which can attack different parts of the body. When bacteria invade the lungs, they cause the most common form of community-acquired bacterial pneumonia; when bacteria invade the bloodstream, they cause bacteremia; and when they invade the covering of the brain, they cause meningitis. Pneumococci may also cause otitis media (middle ear infection) and sinusitis.

Pneumococcal conjugate vaccine is recommended for all children less than 24 months old and for children between 24 and 59 months old who are at high risk of disease. Older children and adults with risk factors may receive the pneumococcal polysaccaride vaccine
Pneumococcal infections normally fall into one of two categories:non-invasive pneumococcal infections - are infections that occur outside the major organs, or blood, and tend to be less serious, and invasive pneumococcal infections - that occur inside a major organ, or blood, and tend to be more serious.
Risk for pneumococcal disease is highest in young children, the elderly, and persons of any age who have chronic medical conditions such as heart disease, lung disease, or diabetes, or conditions that suppress the immune system, such as HIV. Smokers and those in close contact with small children are also at higher risk. Pneumococcal disease is more common in winter months and when respiratory viruses such as influenza are circulating. Outbreaks of pneumococcal disease are not common but can occur in child care centers, nursing homes, or other institutions.
Pneumococci may also cause infections in the ear (otitis media), paranasal sinuses (sinusitis), the tissues covering the brain and spinal cord (meningitis), and, less often, the heart valves, joints, and abdominal cavity. Sometimes these areas become infected because the pneumococci have spread through the bloodstream from another site of infection.
Meningitis is the most severe type of pneumococcal disease. About 5% of children under age 5 who contract pneumococcal meningitis will die of their infection; others can have long-term problems such as hearing loss. Many children with pneumococcal pneumonia or bloodstream infections (also called bacteremia) will need to be hospitalized.
Pneumococci are encapsulated, lancet-shaped, gram-positive diplococci. The bacteria are transmitted person to person via respiratory droplet contact. Pneumococci can cause disease either by direct spread from colonized mucosal surfaces (eg, otitis media) or by hematogenous spread (eg, meningitis following bacteremia). Mucosal irritation resulting from factors such as viral infection or smoke often is a predisposing factor for pneumococcal infection. Ninety serotypes have been identified, with varying degrees of pathogenicity.
The virulence of each organism is determined in part by the makeup and amount of capsule present. In a pneumococcus-naive host (or in the absence of antibody to pneumococcal capsule) host-cell phagocytosis is severely limited because of the inhibition of phagocytosis and the inhibition of the activation of the classic complement pathway. In addition, in vitro and in vivo studies of clinical isolates have shown that pneumococci have the ability to obtain DNA from other pneumococci (or other bacteria) via transformation, allowing them to switch to serotypically distinct capsular types.
